Mostly, devices such as USB headphone and USB card readers are randomly disconnecting and reconnecting on any port. I've tried to disable power saving for the various USB devices/ports and also reinstalled Windows from scratch and the issue is persistent. I hope someone can offer a permanent solution. Thanks!

if the combined power draw of all your usb devices slightly exceeds what your usb ports on the pc are rated for itll slowly heat up the fuse leading to a disconnect of power (subsequent cooling down of the fuse will connect it again) the same applies if you connect multiple devices through an unpowered usb hub.
If it's mouse, try a different mouse. If it's a device or devices connected by a cable (especially micro USB), replace the cable (s). Eliminate things that are easy to fix first. For multiple devices maybe try removing all but one to find out which device has the problem. I would also check the power supply.

All the USB ports will connect, disconnect and reconnect in the period of a second or two, repeatedly. None of them are useable. I've tried reinstalling the drivers, disabling USB 3, enabling and disabling the power option that allows turn them off. Thanks to JimB, I have used Nirsoft's USBLogView and found that one of my Microsoft Hub Ethernet port gets into cycle of connect/disconnect. When I looked close into my setup, I had a wifi usb adapter aswell connected via (Aukey Multi-function USB Hub - CB-H17). The sooner I disconect my wifi USB adapter, the annoying noise stopped.
Scroll down to "USB Root Hub (USB 3.0)," you may have more than one entry depending on how many USB 3.0 ports you have. Right-click it, go to properties, then click the Power Management tab. Uncheck the box that allows the computer to turn the device off to conserve power. Repeat for any other USB 3.0 ports and see if that helps.
